Gentle Transitions Hospice is a local, Nurse, and family-owned hospice agency in Central Texas.  We provide quality end-of-life care for patients and their families in their desired location.  

 

We service:

 60-mile radius of Waco-Temple-Killeen,Texas.

 30-mile radius of waxahachie, tx.

 30-mile radius of southlake & Grapevine, tx.

 

We provide skilled nursing care, physician oversight, social workers, chaplains, certified nursing assistants, and volunteers for our hospice patients.  

 

We are proud to serve our local Veterans as well, as we are contracted with the Department of Veterans Affairs.

Old-Fashioned Hospice

Gentle Transitions Hospice is a locally and family owned business.  The owners are mother and son, Gwenn Dalton and Evan Hessel.  Gwenn has been a Hospice Registered Nurse, Director of Nursing, and Administrator in Central Texas for 25+ years and has a true passion for End-of-life Care.  Evan has worked in hospice marketing and administration for the past 15 years, though he will always tell you that he grew up in hospice, with a deep respect for it.  Gwenn and Evan dreamt that one day they might have their own "Old-fashioned" hospice.  What they meant by that is, after both having worked for large hospice companies and corporate entities, they wanted to return to a time when patient care was the #1 priority, as opposed to only being concerned with the bottom line.  Gwenn recalls when she first started hospice nursing, saying, "The hours were long and the work was hard, but at the end of the day our patients and staff always felt greatly appreciated."  And this is the exact environment that will be encouraged at Gentle Transitions Hospice.  We want to treat our staff with love and care, so they pass on that same blessing to every single one of our patients and their families.  At Gentle Transitions Hospice everyone is family! 

  • Pain re-assessment will be done within 24 hours of any pain medication changes.

  • When a second call from facility staff member, patient, or family member, regarding the same concern or issue is received, a visit will be made unless otherwise requested.

  • A team member will attend visitation or funeral for each patient death.

  • Staff will uphold the rights of the patient and family to grieve in their own unique way and encourage all appropriate expressions of their grief.

  • As hospice caregivers we will each be responsible for establishing and maintaining continuity of care.

  • Gentle Transitions staff will exemplify integrity, honesty, and credibility.

  • Gentle Transitions staff will support the family and caregivers in honoring their goal for the patient’s death to the best of our ability.

  • The case managers will offer a visit the day following admission to hospice care.

  • Gentle Transitions staff will respect all differences regarding beliefs, culture and lifestyle among employees, patients, families, and communities that we serve.

  • Management will cultivate an environment which encourages all team members to convey respectful open communication without reprimand.

The terminally ill deserve the best possible comfort care, and both patient and families deserve the best possible supportive care to relieve the physical, emotional, and social burden of pain and suffering.

Mission Statement

Gentle Transitions Hospice understands that Hospice care is more than tending to the physical end of life; it is a way of caring for the patient, their family, and support group. Through our training and mentoring program of oncoming staff, we will empower those caregivers and end of life professionals with the ability to advance quality of life and to accept the gift of providing a compassionate, dignified, and quality end-of-life for their patient or loved one.

Our goal is to help patients and families live fully and completely, to the best of their ability, one day at a time for the remainder of their lives.
